TSIA-2 Voucher Request Form (SY 26-27)
This form must be completed by the student who will be taking the TSIA. 

The questions on this form will allow us to generate a TSIA-2 testing voucher.  This voucher will be given to you when you arrive for testing. Please make sure you read each question and the responses carefully to avoid problems that may invalidate your test.

TSIA 2 Score Requirements and Test Explanation
The TSIA 2 is a computer adaptave test. Your answers throughout the test determine the level of question you will receive and how many questions you wil be asked. Although we hope you will do your best on the TSIA so you do not have to take it againg, please do not let the test stress you out. It's only a test.
